The Geriatric Care Management Specialists at Sun Family Care Services are here to provide care and support for seniors and individuals who are caring for older adults. We work closely with seniors, their families, caregivers and members of their health team throughout south Florida.
The purpose of our Care Management program is threefold:
- Understanding your needs and the needs of your family
- Developing an individualized, comprehensive plan and
- Monitoring and ongoing evaluation.
We give families peace of mind knowing that someone is watching over their loved ones and available to be there when needed. Our unique Team Approach is geared to address all the needs of the senior in a holistic way. Medical, nutritional, psychological, social, financial and living environment are all considered in developing the Care Plan to prevent crises, when possible, and to manage them when they occur. We also work with caregivers by providing resources to improve the quality of life for all. Our team is available to provide elder care at home, in independent living and assisted living facilities, as well as in other senior living environments.
Our team determines what the older adult needs, coordinates appropriate services and monitors those services to ensure their quality and to improve the quality of life of our seniors and their families. Our goal is to provide caring, compassionate care provided by professionals you can trust. Whenever home health care or companion services are needed, Sun Family Care can be there to coordinate, monitor, problem solve and assure timely communication with the family.
We serve families throughout Broward, Miami-Dade and Palm Beach counties, and provide referral services in other locations.
