We are pleased this month to bring you an exclusive interview with Marc Agronin, M.D., a board-certified geriatric psychiatrist and the Director of Mental Health and Clinical Research at the Miami Jewish Health Systems. Dr. Agronin has written a book entitled, “How We Age: A Doctor’s Journey into the Heart of Growing Old,” that contains remarkable stories of vitality, connection, and growth that transformed his own view of aging. 

Dr. Agronin recently appeared on The Today Show with Ann Curry and Dr. Nancy Snyderman. He graciously answered some of our most pressing questions, and his answers may surprise you.   

Sun: In the book, you describe your emotional reaction to the decline of your grandfather as vacillating “between denial and despair.” As Geriatric Care Managers, we frequently see this in family members. How do you help families and caregivers balance reality with hope? 
Dr. A: Our first priority is to meet people at their level. Some people survive on denial and, unless there is an immediate issue of safety, it may be risky to challenge it. That said, it is important to take some time to get to know the patient and the caregiving network. I was treating a patient with mild cognitive impairment and paranoia, with a lot of chaos at home. I spent the first 30 minutes just talking with him about where he came from. In fact, we took out a map and pinpointed his birthplace. By doing this I was able to connect with him and develop a level of trust. The most important time we spend may be the first 20-30 minutes, making an effort to get to know the people involved. If a  caregiver is clinically depressed, the initial focus may have to be on the caregiver.

Sun: We love your belief in people’s ability to change and grow, even into their 90’s and beyond. But what about older adults whose behavior seems to create distress in their adult children?
Dr. A: A key responsibility is case formulation; not just diagnosis, but underlying causes and dynamics. So we have to be flexible in our approach – there is not a one-size-fits-all method. We have to be prepared to tailor our approach, depending on the patient’s needs.

Sun:  What are some of the factors that can have a positive effect on patient outcomes?
Dr. A: When I start my day I’m excited to meet the people who are going to come into my office. I want to be helpful to them; I’m passionate about my work. And patients and caregivers realize it. I believe that having someone genuinely interested in them is part of the healing process. Evidence-based practice is important, but it’s only one component. Caring for the elderly is so complex, even the most sophisticated caregiver is easily overwhelmed. That’s why 
 
Geriatric Care Managers can play a critical role. The outcome is always better when a Care Manager is involved.

We are approved Medicare providers of Counseling and Psychosocial Evaluation. HELPING FAMILIES WITH TRANSITIONS 
Think of us as your guide through the complicated health and senior services maze. 
Our services include:   

*Accompanying the client to the hospital, as an advocate and someone who knows the person, their wishes and those of the family, their healthcare needs and preferences. 

*Helping the family select the most appropriate rehab facility or arranging for care at home.   

*Researching the independent or assisted living community that will be the best fit for the person, taking their personality and lifestyle into account; ensuring a smooth transfer.   

*Helping the family accept the need for a higher level of care when the person can no longer function appropriately in independent or assisted living, and facilitating the move.   

*Providing evaluation and counseling (through Medicare, at no cost to the client) for emotional or psychological distress.   

*Providing support and counseling for family members whose loved one is declining.

Our next community event is a program for Case Managers and Discharge Planners at Broward General Medical Center with Alice Reiter Feld, Attorney at Law. The program will be: "Hospital Discharge Planning: Legal and Psychological Factors."
